Arrival at East Park Care Centre
What happens when a resident arrives at East Park Care Centre?
Upon admission each resident, where possible, will contribute to their individual plan of care. This will include a description of the resident's preferred daily routine, likes and dislikes with food and drink, and details of any risk assessments taken.
It contains preferences in how they like to be addressed, and what dignity, privacy and respect means to them in terms of their daily activities and behaviour. It also includes any care needs, medication, GP details and whether community nursing care is required. It will also include preferred social activities, religious beliefs and how these will be met.
Each resident is allocated a Keyworker to liaise closely with in term of care planning, as well as social activities.
